Welcome To The NVD Health Office


School nursing is a specialized practice of nursing, which promotes and protects student health, wellness, and disease prevention. The School Nurse combines health care and education and ultimately advances academic success.

The School Nurse works closely with students, families, and staff to ensure all students are safe and ready to learn. Services include but are not limited to, preventive health by providing health maintenance education, health screening, necessary therapeutic intervention, and care of sick and injured students and staff.
The School Nurse assists students and parents in the management of health needs while in school and the coordination of care in the home, school, and community.

To maintain the health and well-bei
ng of your child and other students, a student needs to be kept home to recuperate for twenty-four hours after being diagnosed with a communicable disease such as stomach virus, strep throat, or having a fever of 100 degrees and above
.

Entrance Requirements:

  • New students must submit to the nurse's office a current immunization record.
  • If you are coming from another country or another state, a complete physical must be completed within 365 days prior to entry.

The school Nurse will review all immunizations. If a student requires additional immunizations, they will be given a 30-day grace period to obtain those immunizations. If the immunizations are not given, the student will be excluded from school.

*Please contact the health office if there is a chronic medical condition of which the Nurse should be aware. Accommodations can be made to help your child in school as needed. Make sure to update the Nurse of any new allergies (food or medicine) or medical conditions affecting your child. It is important for the Nurse to maintain accurate medical records.

Physical Forms: 
If possible, please submit a physical form whenever your child has a physical to keep our records current.

  • The Universal Health Record and Sports Physical forms are both good for one year. note: Universal Health Record forms are accepted but are not accepted for Athletics.

    Sports Physical Forms:
  • All four pages must be completed.
  • The student-athlete and parent/guardian must sign at the bottom of page 1.
  • The physician, physician’s assistant (PA), or nurse practitioner (NP) must complete and sign pages 3 and 4. There must be an official office stamp on page 4. 
  •  All physicals must be originals from within the last year (365 days) 
  • Scanned, emails or copies are not accepted & include all immunizations that your child has received when submitting physical forms.

Health Screenings: All screenings are state required and conducted bi-annually.

  • All students will have their height, weight, blood pressure, vision, hearing, and scoliosis screening services provided.
  • Screenings are performed during your child’s scheduled Wellness day. Referrals will be sent to the parent/guardian as needed

*Parents may request that their child not have in-school screenings. If you choose not to have your child screened, please send a written letter or email to the school Nurse. In addition, we have a declination form for scoliosis screenings (click here for the form).

Medications: Should it become necessary for your child to take the medication in school, the nurse must be notified. The school Nurse may only administer medications in school with a physician’s order and parental permission.

  • All medication must be received by the school nurse and delivered in the original packaging and/ or prescription.
  • For prescription medication: we must comply with state laws and will need to contain, as a minimum, the student’s name, prescriber, medication name/dose/directions, date of prescription, and additional comments pertinent to storage, administration, or stability (e.g. protect from light, shake well expiration date).
  • For over-the-counter medication: must be received in the original container and sealed with a label having the student's name affixed.

*Students are not allowed to have or carry any medication in their pockets, backpacks, or lockers, unless a physician’s order is on file which indicates that the student may self-medicate with an inhaler, Epipen, Glucagon, or insulin.
*Students are never allowed to give another student any medication.


Accidents / Injuries while in school:
If your child was injured in school, the school Nurse must be notified as soon as possible. A school insurance form will be completed however, it is the parent/guardian’s responsibility to complete and submit the remaining parts of the form. If you have any questions or concerns, please call the school Nurse.

  • If your child was hurt while participating in athletics, please speak with our Athletic trainer.

Any delay in this matter may eliminate the insurance coverage and place the student at risk.

Crutches and Casts: Should your child require either crutches or a cast your child must report directly to the nurse with a doctor's note indicating:

  • Diagnosis
  • length of time to be excused
  • excuse/ restrictions from Physical Education and sports
  • anticipated time of return to normal activity or specific accommodations

The school Nurse is not allowed to excuse a student from school or Physical Education due to an illness without a physician’s order.
